Most programs don't fail at the finish line. They fail months earlier, when governance goes soft, stakeholders drift, and no one flags that the baseline has slipped. That's usually when I get the call.
The fix is rarely heroics. It is structure: a baseline someone actually owns, decision rights people can name, escalation that moves faster than the problem.
